information forms and return them to our office. You may call our
office to request a copy of the forms via mail or print them directly
from this website.
Note: If you are seeing Dr. Lin for the first time but are a patient of the practice, just fill out the history and physical form. If you are a brand new patient to the practice seeing Dr. Lin, please fill out all of the attached paperwork.
